RELEASE AND WAIVER: I give my permission as parent/guardian for the above child to participate in the Alta Vista Walk-a-thon sponsored by the Home & School Club. I understand that AVHSC and Alta Vista School are not responsible for the supervision of my child(ren) during the event. All children MUST HAVE AN ADULT ON THE PREMISES while they are walking. I realize that this activity is voluntary and not part of the Union School District program. I acknowledge that I understand and assume all risk of bodily injury to my child(ren). Children may not ride bicycles or scooters on campus during the Walk-a-thon event as this poses a safety hazard. I understand that my child(ren) is/are expected to follow the course rules and will be disqualified from prizes should they engage in dishonest conduct such as cutting corners or lap padding. I also acknowledge that photographs or videos may be made of the event and grant Alta Vista Home & School Club specific permission to reproduce, publish, and use any and all such photographs or videos for school purposes. By clicking on the "Submit Registration" button below, I signify that I have read and understood this permission slip. I also agree to provide a signed permission slip, in the event one is requested of me at the event.

Also, when you click "Submit your registration" you will be redirected to PayPal to pay your initial $40 flat donation online. Your $40 flat donation counts towards your overall fundraising goal and entitles you to a t-shirt, lap treats, and lap feet on the day of the walk-a-thon. You will not be able to create your personal online pledge page until you have paid the initial flat pledge online.

If you do not wish to pay an initial $40 flat donation online, please contact Anne-Marie Hart at to manually activate your student's personal pledge page. A of minimum $40 in flat donations/pledges must be met in order to receive a walk-a-thon t-shirt, lap treats, and lap feet on the day of the walk-a-thon.
